We do ask that patients ask their primary care physician for a referral to our clinic, and that any diagnostic or treatment records for pain be obtained for physician review. This is to prevent unnecessary duplication of testing or treatment.
The clinic is located on the first floor at Advocate Sherman Hospital. Our physicians are not employees of Advocate Sherman Health Systems, but have been granted privileges to practice at the hospital. It is helpful to verify with your health insurance company that both the hospital and physician group are in your plan before making an appointment.
Our hours are Monday-Friday 9:00-5:30. Our phone number is 224-783-8976. Please call us if you have any questions about the services we offer or how to access our clinic.
